The medicines, which increase the tone of vascular wall, just as vasos-dilatator, depending on the place of the application of effect and special features of the mechanism of action are subdivided into several groups.
1. The medicines, which realize their action predominantly at the level CNS:
a) psycho-stimulators - caffeine and other;
b) analeptics - nikethamide and other
2. The medicines, which realize their action predominantly at the level of the peripheral nervous system:
a) the stimulating α- adrenoreceptors - noradrenaline, phenylephrine and other;
b) stimulating α- and β- adrenoreceptors - adrenaline, ephedrine and other;
c) stimulating α-, β- and dopamine receptors, dopamine.
3. The medicines of myotropic action (stimulating angiotensin receptors - stimulators of AT1- receptors)) - Angiotensinamide.
Stimulator of AT1- angiotensin receptors - preparation of Angiotensinamide (sin.: hypertensin), in the essence, is the exogenous analog of the endogenous biologically active compound of angiotensin of the II, which, as it was noted, is main effector element RAAS. Therefore in angiotensinamide are inherent all effects of circulatory RAAS. Preparation renders the expressed pressor (increasing pressure) effect, which in the intensity 40 times approximately exceeds the same in noradrenaline. The pressor effect of angiotensinamide realizes by the stimulation of AT1- receptors, located on the cellular membranes of the smooth musculature of vessels. Besides this the specific contribution to the pressor effect of angiotensinamide introduces the ability of preparation to increase the activity of vasomotor center and the sensitivity of a- adrenoreceptors to the noradrenaline.
Under the effect of angiotensinamide to the greatest extent are reduced the arteries of the skin and internal organs. However, preparation practically does not influence to the coronary blood circulation, the arteries of skeletal musculature and the tone of veins.
Just as angiotensin of the II, Angiotensinamide stimulates the production of aldosterone, which entails the delay of the ions of Na+ and water in the organism and it, therefore, also contributes to the lift of arterial pressure.
In the plasma of the blood the preparation rapidly (during several minutes) is metabolized; therefore for achievement steadfast lift AD it is introduced intravenously in the form a constant infusion.
Uses Angiotensinamide for lifting the arterial pressure with the vasomotor collapse, post-traumatic, postoperative, cardiogenic shock, thromboembolism of pulmonary artery.
