Description
Active substance - sodium nitroprusside (Natrium nitroprussid): Sodium nitrosylpentacyanoferrate. Freeze-dried form of nitroprusside of sodium in the ampules with the stabilizer (sodium citrate).
Pharmacological action
Preparation renders the expressed vasodilator action, caused by direct influence on the smooth-muscle cells of the walls of vessels. As a result action on the walls of arteries AD it is reduced and thus is reduced postcardiac load.
Indications for use
Crises are hypertonic of any origin, malignant, hypertonia; the sharp left-side insufficiency of heart with the myocardial infarction.
Method of application and dose
It adapts exceptionally in the form drop infusion into the vein, before this the lyophilizate in the ampule dissolve 5-10 ml 5%- foot of the solution of glucose or 5 ml of the distilled water. They fill the prepared solution into the vessel with 150-500 ml 5%- solution of glucose. The speed of introduction is determined by the necessary or already achieved result. Initial optimum dose 0.5-5 mg/kg of the mass of body (typically 2.8 mg / (kg min)). Only freshly prepared solution is used, mixing with the solutions of other medicines is forbidden.
Side-line action
From the side of nervous system and sensory organs: noise in the ears, myosis, vertigo, nervousness, anxiety, muscular twitchings, hyper-reflection, engine uneasiness, an increase in the intracranial pressure. CO of the side of cardiovascular system and blood (homopoiesis, hemostasis): excessive hypotension, the phenomenon of “rebound” (heavy hypertension) during the rapid curtailment of infusion, tachycardia, bradycardia, change on EKG, methemoglobinemia, the decrease of the aggregation of thrombocytes. On the part of the digestive tract: abdominal pain, including in the stomach, bowel obstruction. Other: hypothyroidism, headache, sweating, pain or redness at the injection site, hyperemia of face, skin rash.
Contraindications
Hemorrhage into the brain, the disrupted metabolism of cyanides. Caution with the presence of arteriovenous anastomosis and increased intracranial pressure is necessary.
Form of the release
Ampules on 10 ml of the dark glass, which contain 30 mg of nitroprusside of sodium. In the packing to the ampule they apply the ampule, which contains 5 ml of solvent.
